Medical
Customize your healthcare needs with either a PPO or HMO for you and your dependents, which include spouse, child, common law and domestic partner.
PPO

* Allows greater flexibility and coverage alternatives
* Offers a network of providers through Humana
* See any provider and still receive limited benefits
* Higher paid benefits offered at an HCA facility
* Deductibles and coinsurance apply
* Build the plan that's right for your situation by adding optional riders to your base coverage

HMO

* Offers comprehensive coverage through a network of providers and facilities through Humana
* A nominal co-pay per office visit/admission
* Discounted vision and dental coverage

No Coverage Option
If you are a full-time employee with coverage elsewhere, you can elect "No Coverage" and receive Cash-Out Dollars.

Dental
Optional dental coverage with a PPO or a DHMO:

* Offers in- and out-of-network benefits through MetLife
* Deductibles and coinsurance apply

DHMO
CompDent and CIGNA both offer discount options with network dentists without the hassle of deductibles and claim forms based on a discounted fee schedule.

Vision
Plan provisions offer a co-pay for eye exams and an allowance and discount on frames, lenses and contacts, and a discount on laser vision correction with an EyeMed network provider.

Flexible Spending Accounts
Health Care and Day Care Spending Accounts provide a way to pay for certain health care and day care expenses with before-tax dollars. The advantage is reduced tax liability, which means savings. Employees may participate in one or both of the accounts.

Life and Accident Insurance
Methodist Healthcare offers Basic, Supplemental and Dependent life and accident coverage to help provide financial security in case of death or serious injury.

Short-Term/Long-Term Disability
Methodist Healthcare offers STD after a 45-day waiting period from the date of disability. LTD coverage is offered after 5 months of disability to help provide financial security after a serious accident or illness that prevents the employee from working.

RETIREMENT AND SAVINGS

Retirement Plan
The Retirement Plan is the company's main contribution toward helping the employee achieve retirement goals. The Methodist Healthcare contribution of 2.25% up to 5.5% to the employee's account is based on salary and years of service. Employees are fully vested after 7 years of service.

Salary Deferral 401(k) Plan
The 401(k) savings plan is tax deferred (income before taxes), meaning the employee does not pay taxes on the savings contributions or their earnings until the funds are withdrawn.

* Employees can contribute from 1% to 50% of their pre-tax earnings up to the IRS maximum
* Matching contributions up to 50% of the first 3% that the employee saves
* Employees are fully vested after 3 years of service

EDUCATION


Tuition Reimbursement
Methodist Healthcare offers a tuition reimbursement program that provides financial assistance for courses taken towards a degree at an accredited university or college. This program reimburses 80% for tuition fees and book fees up to a maximum of $500 per semester ($1,500 annually). Educational Loans are also offered up to $500 per semester, and repayment is made through payroll deduction during the semester.

Scholarships
Optional scholarship programs are offered by Methodist Healthcare to provide support for employees pursuing a health career.

Additional scholarships offered through HCA to Methodist Healthcare employees and dependents are as follows:
Health Careers Foundation (HCF)
The HCF program is designed to assist adults who want to either enter or change careers in certain medical fields of study. Recipients can receive up to $4,000 annually.
HCA Foundation
College scholarships for dependent children who are high-school seniors, or college freshmen, sophomores or juniors are eligible to apply. Awards vary from $500 to $4,000 annually.

Professional Development
Methodist Healthcare has an extensive educational department and staff offering a variety of courses that will assist staff members with competency requirements and CEU credits. Methodist Healthcare also provides a means for ongoing reassessment of educational needs in the maintenance and improvement of competency skills by providing a designated educator for each unit. Additional courses are also offered to assist in career advancement.

TIME OFF

Paid Time Off (PTO)
Methodist Healthcare offers a Paid Time Off bank that includes vacation, holidays and sick time to allow maximum flexibility for the employees Employees accrue hours every pay period based on authorized hours worked and can start earning as much as 22.33 days per year. PTO hours may also be cashed in at 90% of their base rate at certain times during the year.

EXTENDED ILLNESS

Extended Illness Bank (EIB)
Designed to replace earnings in case of an accident or illness after using 24 hours of consecutive PTO. Employees accrue hours every pay period based on their authorized hours worked and can start earning as much as 7.67 days per year.

Adoption Reimbursement
The Adoption Plan pays for a portion of your adoption up to $3,500 for expenses you have within the plan year relating to the adoption of a child (non-relative).

Employee Assistance Program
A free confidential assessment, counseling and referral program, which helps employees and their families solve problems that may affect their health, family life and/or job performance.

Employee Wellness Program
Program promoting a healthy lifestyle for the employee. The Wellness Program provides reimbursement for certain expenses associated with health and fitness. Reimbursements range from $75 to $375 annually based on certain criteria.
